The person is aware of discomfort from pressure and may move to prevent pressure sores. Ensure adequate pressure care by careful observations of their skin integrity. Treat pressure sores with appropriate pressure relieving methods such as regular repositioning and use of pressure relieving devices.

Common pressure relieving devices are:

  • Pressure care mattresses with pressure relieving foam or a static pressure care mattress may also be used if they lack other risk factors such as decreased sensation, poor circulation and diabetes mellitus.
  • Medical grade sheep skin sheet or heel protectors for bony prominences.
  • Bed cradles to prevent heavy blankets causing unnecessary pressure.
  • Seating pressure care cushions such as Jay2 and Roho cushions. Consult a seating specialist for other options.

Pressure sores are graded as:

  • Grade I – skin discoloration, usually red, blue, purple or black.
  • Grade II – some skin loss or damage involving the top-most skin layers.
  • Grade III – necrosis (death) or damage to the skin patch, limited to the skin layers.
  • Grade IV – necrosis (death) or damage to the skin patch and underlying structures, such as tendon, joint or bone.
